Like all of our surveys, it is completely voluntary whether or not you choose to take part in this survey.
If you gave a preference for completing questionnaires online, you will receive an email with a personalised web link and a unique ID.

You can complete the question using a PC, laptop, tablet or smartphone using either the link in the above section or the link in the email. You will need the unique ID which can be found on your letter.
We recommend you complete it all in one go. It should take around 45 minutes to finish. However, if that is not possible, you get back into the questionnaire using the same login details.
As always, answering these questions is voluntary and you can skip over any questions that you cannot or would prefer not to answer.
Once you have come to the end of the questions, you will be required to submit your answers. Once the answers are submitted you won’t be able to edit them again.
You only need to complete one version, either the postal or the online version. Please do not complete both.
The same questions are asked on both the postal and online versions.

The information you give us will be held securely and treated in strict confidence in accordance with the Data Protection Act 2018 and the General Data Protection Regulation. Any information that may identify you, such as your name, your contact details and the contact details of anyone else you give us, will be kept separate from responses to any other questions you have answered. We hold any information which could identify you securely and never make this available to researchers.
